At Polk City Chiropractic, our focus is on you.
During your first visit to our office, Dr. Chris will review a history of your health and current condition, and perform a series of orthopedic, neurological, and spinal exams to formulate a diagnosis and see if Chiropractic care is right for you. If so, you may receive your first adjustment that day to begin the healing process immediately.
On the second visit to our office Dr. Chris will clearly explain your condition, the exam findings, and what we can do to help. We will discuss our recommendations for care and your health care options, and allow you to decide which course of action to take.
Financial Policy and Insurance Coverage
We gladly accept most major health insurance plans in our office, including but not limited to: Blue Cross/Blue Shield HMO (Blue Access, Blue Advantage, Blue Choice), Blue Cross/Blue Shield PPO (Alliance Select), United Healthcare, UMR, Cigna, Hawk-I, Medicare, and Medicaid. Please contact us if you are unsure of your insurance coverage and we will be happy to help!
Most insurance companies provide varying amounts of chiropractic coverage, and the patient is typically responsible for a copayment, co-insurance, or deductible payment. These and payments for non-covered services are due at the time of service unless other arrangements are made. We can verify your coverage with your insurance company.
